Admission ApplicationComplete and Submit the Admission Application. Also, a one time fee of $50 should be submitted with the Admission Application for Admission for New Students to Covenant Academy.

Annual Enrollment  / ReEnrollment Application
-  Annual Enrollment is necessary to Enroll new Students or to Reenroll existing Students in Covenant Academy. Complete and Submit the appropriate form for each Student each school year.  Enrollment and Reenrollment Tuition fees are $150 per student/family per school year.

Church School Enrollment Form  -  All Students must Complete and Submit the Church School Enrollment Form to Covenant Academy for each Student Enrolled every year.  The school will mail these to school superintendents in each district.

Request for Records  -  When a Student is enrolled after attending another school, we will request records from that school.  Those records will be placed in our file.  When Students leave this school, all information received from the previous school will be forwarded to the new one along with records provided.

Attendance Records 
-  Attendance Records must be completed for each Student.   Attendance Reports will be due to Covenant Academy by the fifth day of December, March, June and September (for summer school) for the previous three months.

Report Cards  -  Parents can complete report cards use the Report Card form.  School staff will be glad to sign, stamp and emboss report cards sent in by parents, but they are not to fill them in.

Transcripts 
-  Transcripts shall be completed annually.  The school office staff WILL NOT complete or prepare transcripts, they will, however, be glad to sign, stamp, and seal and mail as needed.  When a school requests records from Covenant Academy, they will be sent a copy of whatever the parent has put in our file.  It is up to parents to see that this material is current.  Transcripts for Graduates will be prepared by our staff and sent as needed.  There is a $5.00 charge for each transcript after the first two.

-  Complete and Submit Cumulative Records to Covenant Academy.

IEP (Individual Educational Plan) 
-  Students with diagnosed learning disabilities or other special needs should be provided with an IEP (Individual Educational Plan) for each school year.  These will be completed by the parents with assistance from school staff as needed and will become a permanent part of the student's file.

Work Permits 
-  Application for these may be obtained through the central Montgomery of Covenant Academy or from any local school district office.  The section for school attendance must be filled out and signed school staff member.  The Student must personally turn in the application to the same office where it was received, and show positive ID before the actual permit will be issued.

Drivers License 
-  The form needed to prove school enrollment for obtaining a driver's license is the Church School Enrollment Form for the current school year.  Be sure these are signed by an appropriate school official.

-  The Covenant Academy Diploma provides for Student Graduation and Diploma presentation.

Accelerated Diploma Program   The Accelerated Diploma Program is for students in the regular church school program or a traditional school.

Adult Program  The Adult Program is for older Students who have not graduated from high school for whatever reason.

Notice of Withdrawal  -  The Notice of Withdrawal form must be completed within one week after a Student ceases to attend our school or enrolls in another one.  Students who leave during the school year without withdrawing will be charged $25 before any records will be sent to the next school.
